In past years, over 50 percent of all Informatics majors reported having participated in research, public service, and/or internship activities. Informatics majors have also been recognized in various capacities with awards, scholarships, and special achievements. We’d like to capture all of these! Your reports of involvement and achievement, taken together, illustrate the strength of our program and our students.

The information on research and public service involvement of the Informatics majors will also be forwarded to the Provost’s Office for the University’s annual reporting of the Accountability Data to comply with requirements for the state legislature and the HEC Board. Your name and UW ID number is requested to ensure that the information you provide is verifiable.

No personally identifiable information will be made public without your consent.

The Information School faculty may also review this information when they consider students for School-based Scholarships and award recognitions.
